Residential & Independent Housing Services
MBW Company provide services on a 24-hour or hourly basis. These services are designed around each individual and may include assistance with health and personal care, safety, nutrition, finances, activities of daily living, and training to independently perform these activities. 24-Hour Residential Services
MBW Co. provides both 24-hour supervision, support, and training to people sharing a home (three or four individuals per home) in the community and also provides support services on an as needed, hourly, basis to clients in their private homes. Gleseners Assisted Living Services
Gleseners Assisted Living Services provides services 24 hours per day. This program is structured for adult survivors of traumatic brain injury. Individual & shared apartments are available. An emergency call system is available for anyone requiring special immediate staff attention. Tenants who meet income guidelines are eligible for Section 8 rental assistance. This program is only offered in Bird Island, MN
Adult Day Services
The Adult Day Program offers a supervised, structured environment to enhance knowledge and awareness. Consulting professionals are also available. This program is specifically designed for adult survivors of traumatic brain injury. The program helps maximize the physical and emotional health of participants by providing physical exercise, health and nutritional supports. We take an innovative approach specific to each individual served. This program is only offered in Bird Island, MN
Hourly Support Services
MBW Co. also provides support and training on an hourly basis, which supports the individual to live in a least restrictive environment. Services are designed to promote independence and encourage success. Hourly support is provided through Semi-Independent Living Services (SILS), Waivered Program Services and our Senior Support Services.
Home Care & Nursing Services
This service provides basic health care and daily-living support in the individual’s home on an as needed basis, up to 24 hours per day, seven days per week.
Individualized Housing Options (IHO) Services
This service provides a supported apartment environment, which in conjunction with the 24 hour Response Service can provide direct support to you in your own apartment or with a roommate who also receives services. This allows the independence and privacy in your own apartment.
Services and Fees
Waivered services, including DD, CADI, CAC, BI and EW, are designed around the individual and may include assistance with health and personal care, safety, nutrition, finances, activities of daily living, and training to independently perform these activities.
Fees will vary depending on the service and site. Service costs may be covered through the Medicaid (MA) program, the client’s county of financial responsibility or by payment directly from the individual.
